The best evidence that the enlargement of the cardiac silhouette is due pericardial effusion is the demonstration of serial change [either increase or decrease (with treatment)]. The image below demonstrates a smaller cardiac silhouette after clearance of pericardial effusion which was documented by echocardigraphy.
