Anterior wall myocardial infarction, right bundle branch block and left anterior hemiblock. QR complexes are seen in V1 and V2. The initial r wave in V1 has been knocked by infarction so that right bundle branch block (RBBB) manifests as QR pattern in V1. This pattern is also called QRBBB. Left anterior hemiblock (LAHB) is identified by the left axis deviation with rS complexes in inferior leads. In addition there is sinus tachycardia.
