Non ST elevation myocardial infarction (NSTEMI) cannot be fully diagnosed from ECG alone because a similar pattern can be seen in other conditions as well. So a clinical history suggestive of myocardial infarction and elevation of biomarkers of myocardial necrosis are needed for a firm diagnosis of NSTEMI. Hypertrophic cardiomyopathy is an important differential diagnosis of NSTEMI on an electrocardiogram. Myocarditis is another condition which can simulate NSTEMI, in almost all features.
