QS complexes in leads II, III and aVF, with ST segment elevation and T wave inversion indicating a fresh inferior wall infarction. In addition there is ST elevation in V2 and V3. This could represent additional involvement in anterior wall. But there is notch at the J point and the ST segment elevation is concave, in V2, indicating the possibility of pre-existing early repolarisation syndrome.
